Beginning January 1, 2014, this expansion established new coverage pathways for millions of uninsured adults previously excluded from Medicaid. The law also provides that expansion projects will receive 100 percent federal funding through 2016, tapering to 90 percent in 2020 and in future years. However, the Supreme Court’s June 2012 ruling on the ACA gave states the option to expand Medicaid.

Over the past 25 years, many states have taken steps to expand coverage for children. For example, while federal law requires states to provide Medicaid to children at least 100% FPL (133% FPL for children under 6 years old), as of January 2015, 27 states (including Washington, D.C.) have expanded eligibility to At least 255% FPL under Medicaid or CHIP, with 19 states having eligibility limits higher than 300% FPL. Medicaid and CHIP currently cover more than one-third of children, and these programs have an even greater impact among low-income children and children of color. As of December 2014, 29.1 million children were enrolled in Medicaid and CHIP, the vast majority of whom were enrolled in Medicaid. 3

The situation is very different for adult Medicaid. In 2013, before the ACA Medicaid expansion took effect, median Medicaid income eligibility was capped at 61% of FPL for working parents and, in most states, nonelderly adults without children (“childless adults”) Absolutely not eligible for Medicaid. While expansion states offer Medicaid up to 138% of FPL for adults, as of January 2015, the median Medicaid income limit for parents in non-expansion states was only 46% of FPL (approximately $9,200 for a family of three in 2015 ). , and childless adults remain ineligible for Medicaid in all but one nonexpansion state (Figure 4). 8 Because the ACA provides Medicaid eligibility for low-income adults, it does not provide financial assistance for purchasing marketplace coverage for the following groups. 100% FPL.
